LunarIPS is probably the best to go.
I have patched many, many roms and the only times that it produced bad roms were when I had to apply a header to a rom (and everytime that happened I was told to do it in the readme of the patch anyway.
Zophar's Domain has just suddenly announced a new IPS patcher. I haven't tried it, but it may be good. It can apply and create both IPS and NINJA files.

PPF - 32 bits.
NINJA - 2040 bits (still, php bumps it back to 32 bits, and even asm can't bring it above 80).
