Update graphic drivers.
or
Simply change video modes.
or
RTFM.

I am still unsure what video cards are still causing this problem.

I think it happens on Nvidia cards, although I could be wrong.
Try out CCleaner and other free software at Piriform

It happened to one of my computers with a cheap Intel Integrated Video Chipset.

I remember it occurring on a cousin's comp, but I don't remember what card or brand they had. It might have been ATi.
Everything I say is a lie.

It happens on my parents computer too, Matrox Milennium II PCI video card. Never was a big deal though, since it doesn't affect the screen mode I set on it.
Works fine on my computer, GeForce4. So it isn't necessarily NVIDIA, although I do keep my drivers updated.

It happened to me before... Found two ways to fix it.
1- Change video mode under ZSNES
2- Switch from 32-bit colors to 16-bit
I have a SiS video card... Integrated in my motherboard...
